2011 COMMUNITY HEALTH ASSESSSMENT
AND COMMUNITY HEALTH IMPROVEMENT PLAN
From June 2010 through June 2011, the St. Johns County Health Leadership Council completed a comprehensive, county-wide Health Assessment using a nationally recognized approach, called MAPP (Mobilizing for Action through Planning and Partnerships). The Council used the data collected in the County Health Assessment to develop a Community Health Improvement Plan.

It is with immense pride that I serve, and address the public health concerns of the residents of St. Johns County. As our Senior Leaders began the Strategic Planning cycle earlier this year, we reviewed Healthy People 2010 National Health Objectives on Health Promotion and Disease Prevention, and looked toward the launching of Healthy People 2020 next year. As we looked forward, we recognized that in order to achieve Healthy People 2020 objectives, we need to acknowledge and address the social, environmental, and economic determinants of health, including issues such as, crime, poverty, and community planning. Consequently, we will work to ensure that our strategic planning initiatives involve all sectors of our community, and that health implications of decision making will take into account issues which, at first glance, may not appear to impact directly the health of the community.
When I began my tenure at St. Johns County Health Department (SJCHD) in 2004, emphasizing the core values of respect, loyalty, trust, accountability and compassion, and adherence to the strictest of ethical standards, SJCHD embarked on a Journey of Continuous Quality Improvement, embracing the Baldrige / Sterling Management Model to pursue Organizational Performance Excellence. The national Baldrige Criteria for Performance Excellence provide a framework by which SJCHD ensures fiscal sustainability through improved efficiencies, via continuous performance improvement. I am pleased to share that SJCHD has won the 2009 Governor’s Sterling Award (GSA)!
When the program is launched in 2011, SJCHD plans to seek voluntary national Public Health accreditation. The Public Health accreditation domains link to the Sterling Management System, the continuous quality improvement infrastructure which will assist SJCHD to become one of the first Health Departments in the nation to achieve national accreditation. The bottom line is that none of this would be possible, were it not for the commitment and dedication of my staff, Florida’s Best, in your Center of Excellence in Public Health!
