The St. Johns County Health Improvement Council is comprised of various community entities, which make up the St. Johns County Public Health System. Championed by the St. Johns County Health Department, the Health Improvement Council completed a comprehensive Community Health Needs Assessment in 2008, as recommended by the National Public Health Performance Standards Program. The information that was obtained during the six-month assessment process was reviewed, and four key issues were identified:
- Improve access to services in underserved sectors of St. Johns County
- Expand primary education and prevention for obesity and how it relates to chronic disease (Diabetes, Heart Disease and Stroke)
- Improve linkage, referral and coordination among existing Health and Social Service Programs
- Improve availability and access to adult dental services for low-income and uninsured residents of St. Johns County.
The Council has developed goals and activities to address these key issues. At quarterly meetings at the St. Johns County Health Department, members are provided with status updates, and continue to work towards improving these community health indicators.
